| Placement Exam Letter | This letter was mailed home to all incoming 9th Grade Students regarding procedures for the Placement Exam. |
| Adv. Foreign Language Placement Exam | This form should be filled out by all incoming 9th Grade Students. This form should be returned on the day of the placement exam. |
| New Admission Medical Form | Required for all incoming 9th Graders. |
| Placement Student Information Sheet | This form should be filled out by all incoming 9th Grade Students. This form should be returned on the day of the general placement exam. |
| Transportation Application | This form should be filled out by all incoming 9th Graders who are seeking a student MetroCard. |
| Fine Arts Preference Form | This form should be filled out by all incoming 9th Grade Students. This form should be returned on the day of the placement exam. |

| 2009 - 2010 Photography Release | Required for all current students. |
| 2010 Senior Trip Permission Slip | This form must be filled out by all Seniors attending the 2010 Senior Trip. |
| 2009 Philadelphia Junior Trip | This form should be filled out by all juniors attending the Junior Class Trip on October 30, 2009. |
| 2010 Senior Prom Permission Slip | This form must be filled out by all Seniors attending the Senior Prom. All completed forms should be returned to Mrs. Fusco. |
