Admissions
Application for Admission	This form should be used by all incoming 9th Grade students who have been accepted into St. Edmund Prep, all transfer students, and all International Students
Statement of Financial Support	Must be submitted by International Students Only.

 

Athletics
Annual Athletic Physical Evaluation Form	Must be completed by all student athletes at the beginning of each school year.

 

Incoming Freshman
Placement Exam Letter	This letter was mailed home to all incoming 9th Grade Students regarding procedures for the Placement Exam.
Adv. Foreign Language Placement Exam	This form should be filled out by all incoming 9th Grade Students. This form should be returned on the day of the placement exam.
New Admission Medical Form	Required for all incoming 9th Graders.
Placement Student Information Sheet	This form should be filled out by all incoming 9th Grade Students. This form should be returned on the day of the general placement exam.
Transportation Application	This form should be filled out by all incoming 9th Graders who are seeking a student MetroCard.
Fine Arts Preference Form	This form should be filled out by all incoming 9th Grade Students. This form should be returned on the day of the placement exam.
