Incoming Freshmen

Welcome to St. Edmund Prep! As we prepare to welcome your son or daughter in September, there are a few items with which we will need your help. Click on the items below for more details. 

Due Date: Wednesday, May 1, 2024
All tuition payments are managed by our online system, FACTS. We ask that each family begin by creating their FACTS account. Creating this account will not have any immediate financial impact on your family. The first tuition payment is not due until July 1st. Creating your account as soon as possible is important as it takes our tuition office and FACTS a few months to make sure your account is ready for the July 1st tuition payment.

In order for us to create an academic program tailored to your son’s or daughter’s current academic successes and interests, we ask that you fill out this form to indicate your preferences in regards to:
  • Choosing a path in our fine arts program
  • Choosing a foreign language course of study
  • Regent exams taken in middle school

If your son/daughter has an Individualized Education Program (IEP) or is eligible for Section 504 accommodations, submit a copy of that documentation through the document submission portal.

The New York Public Health Law and St. Edmund Preparatory High School policy mandates that any new student must submit ALL the following to attend school:
  1. A completed medical evaluation - Must include height, weight, blood pressure, medical history, a developmental assessment, anemia screening, vision, hearing, dental screening. Please keep in mind that medicals expire one year from the date of exam, therefore, medicals must be up to date before they can be turned in.
  2. Documentation of the following immunizations:
    - 4 doses of Diphtheria vaccine (DtaP or DPT or DT)
    - 1 dose of Tetanus, diphtheria, Pertussis vaccine (Tdap) (given after age 7 years)
    - 4 doses of Polio vaccine (OPV or IPV)
    - 2 doses of Measles, Mumps and Rubella vaccine (MMR) (1 dose given on or - after the 1st birthday, and the second at least 28 days after the first and at or after age 15 months.)
    - 3 doses of Hepatitis B vaccine
    - 2 doses of Varicella vaccine (given on or after the 1st birthday)
    - 1 dose of Meningococcal vaccine (MCV4) (given on or after the 16th birthday – all 12th grade students)
The medical and immunization forms must be signed and mechanically stamped by your physician.  Forms may be returned to the school by mail, or can be submitted through the document submission portal.
 
Only the St. Edmund Prep Medical Form OR the NYC Department of Education Form that your pediatrician can generate from their office will be accepted.
All student athletes will require an additional Athletic Physical Form, completed by your physician, in order to be medically cleared to compete. 
 
If your student will be playing a fall sport, please print out the Athletic Physical Form return it to us by mail or submit via the document submission portal. 
 
Please note: Medical clearance is not given on the same day a Sports Medical Form is turned in. There is a mandatory 24-hour waiting period between turning in the medical and being given medical clearance. Please contact the Athletics Office at this link with any questions regarding the Athletic Physical Form.
Lands’ End is our school uniform provider. You can shop at LandsEnd School for our specific uniform items. Full details of our student dress code can also be found here.
 
We recommend that you place your orders for uniforms no later than June to ensure that you receive them on time. All students will be required to be in full uniform on the first day of school in September.
