All incoming 12th grade students (Class of 2018) must have proof of having been given Meningococcal vaccine (shot) in order to attend school.
Students entering Grade 12 in 2017 (Class of 2018) must have either:
- 2 doses of meningococcal vaccine with the booster dose given on or after age 16
- 1 dose if your child’s first dose was given on or after age 16
Please submit proof of the required vaccine by August 15, 2017 so there will be no delay in your child starting school on time in September. This proof must include:
- Name of the vaccine
- Date vaccine given
- Who gave it, along with their title; or where it was given if at a clinic
Students will not be permitted to start classes in September until Meningococcal Immunization Documentation has been submitted.
